Through recovery-focused outreach support, you'll help people build confidence, reconnect with their community, strengthen daily living skills, and take positive steps towards a life they choose.
In this role, you will provide flexible, person-centred outreach support for up to 12 weeks to adults transitioning from acute mental health services or requiring support to maintain wellbeing and prevent hospital admission.
Working alongside participants, you will support them to build on their strengths, achieve their recovery goals, strengthen community connections, and increase independence and quality of life.
The TRec program operates 7 days per week between 9:00am and 9:00pm.
Job Description
About the Role
Key Accountabilities include but are not limited to:
Assist people experiencing mental illness to have a valued role in their community and enhance their quality of life.
Provide opportunities, support and services to develop or redevelop skills, build confidence and reconnect with the community.
Build supportive, long-term respectful relationships with people experiencing mental illness, their families and carers.
Ensure that services accessed by people experiencing mental illness are coordinated, integrated and complementary to other services in the community.
Provide direct and personalised assistance through outreach services, providing referrals,



and monitoring and reporting progress.
Facilitate access to peer support, mentoring, and group-based psychoeducation activities.
Maintain a professional and welcoming environment through consistent high-quality person-centred practice.
Regularly communicate and document service user feedback to relevant team members and contribute to service improvements.
Contribute where required to service user engagement activities and promote these to participants.
Ability to work within the program operating hours: 9am-9pm Monday to Sunday, year-round (inclusive of public holidays).
This includes completing after-hours participant phone calls.
Willingness to participate in a rotating on-call roster, which includes:
Being available outside of rostered shifts to respond to participant calls.
Provide support or debriefing to staff who are conducting after hours phone calls.
About You
Experience and Qualifications:
Relevant Tertiary qualifications in Psychology, Community Development, Counselling or Social Work preferred.
The minimum qualification for the role is Certificate IV in Mental Health or equivalent.
Minimum 1 years' experience in the mental health sector.
Demonstrated skills in outreach, referrals, risk assessment, safety planning, and reporting.
Strong knowledge of recovery-focused, trauma-informed and person-centred practices.
Ability to build supportive relationships with individuals, carers and families.
Effective verbal and written communication skills.
Demonstrated understanding of professional boundaries, privacy and confidentiality.
Ability to work both independently and collaboratively.
