As a Community CHSP CarePartner, you'll contribute to creating and delivering personalisedsupport services aligned with Carinity's Model of Care, upholdingdignity, wellbeing, and meaningful connection.
You will beresponsible for supporting the operational set-up, day-to-daycoordination, documentation quality, and compliance readiness of acommunity business hub assisting elderly access entry levelCommonwealth Home Support Program services.

You will foster a safe,inclusive and respectful workplace for clients, families, staffand visitors, while actively contributing to service improvementsand supporting the broader goals of the CHSP Community Careteam.
Your day-to-day will involve:
Coordinating client's journey with Carinity, from initialmeet‐and‐greet through admission, ongoing reviews, referralsand discharge
Supporting clients to develop and assess individualisedcare plans that meet each client's approved services, uniqueneeds, preferences and goals
Ensure care services reflect a person-centred,strengths-based approach
Monitor client wellbeing, processing referrals, assessmentsand care reviews.
Liaise with Lifestyle Carers, Contractors and Clients whenrequired, contributing to scheduling, onboarding,
and staffperformance feedback
Build strong relationships with clients, families,assessment teams, healthcare providers and internalcolleagues
Advocate for clients and ensure any concerns or changes inconditions are followed up promptly and appropriately
About you:

You approach every interaction with confidence, warmth andprofessionalism whether guiding staff, collaborating with familiesor coordinating with external providers.
Highly organised anddependable, you seamlessly balance administrative duties withhands-on support.
You communicate effectively with clients,families, staff and health professionals, and you're skilled atmentoring colleagues while fostering a positive, collaborative teamculture.
Additionally;
Certificate III in Aged Care, Individual Support orCommunity Care
Experience delivering or working within Consumer DirectedCare or Care management models
Ability to monitor and support client budget processes
Knowledge of Strengthened Aged Care Quality Standards,SIRS, Aged Care Act ****
Experience using Microsoft Office and care-relatedsoftware
Current NDIS Worker Screening and a current driver'slicence
