Job Description
Key Responsibilities




Deliver family support, parenting education, supported parent groups/playgroups, school readiness programs, and (where applicable) home visiting.
Provide care coordination and case management for families with complex needs - building long-term, holistic relationships.
Facilitate community outreach, education, and development activities to grow local capacity.
Support participants to build skills, confidence, and community connections.
Ensure services are responsive to community needs and integrate with other local supports.
Maintain strong stakeholder relationships for collaborative service delivery.
Keep accurate records, maintain client file security, and apply professional ethics.
Stay current with industry developments and assist other programs/clients as required

Desired Skills and Experience
Skills & Experience (Essential)
Relevant qualifications in welfare or related fields and/or extensive experience working with families with complex needs.
Experience in case management and program delivery - including development, implementation, coordination, review, and evaluation.
Ability to engage with complex families; experience in childcare/playgroups or similar settings.
Experience working with Aboriginal and CALD communities.
Strong relationship-building skills with multi-faceted services.
Current driver's licence and ability to travel regularly (including overnight stays).
