Job Description
We are seeking an experienced and compassionate Community Worker to join our team in Ravenswood.
This full-time position focuses on assessing participants' needs, developing and coordinating support programs, connecting participants with appropriate community services, monitoring their progress and supporting meaningful community participation.
Assess participants' individual needs, strengths, goals and available support resources
Develop, implement and review individual support, community participation and life skills programs
Assist participants to access health, welfare, housing, education, training, employment and community services
Support participants to build independence, confidence, social connections and practical life skills
Organise and coordinate community activities, appointments, recreational programs and social participation
Liaise with participants, families, guardians, support coordinators, healthcare providers, community organisations and government agencies
Monitor participants' progress and identify any changes in their circumstances or support needs
Maintain accurate case notes, progress notes, incident reports, service records and other required documentation
Advocate for participants and help them overcome barriers affecting their community participation and wellbeing
Refer participants to appropriate specialist services when additional assistance is required
Contribute to service planning, program evaluation and continuous improvement activities
Follow all NDIS Practice Standards, workplace policies, privacy requirements and safeguarding procedure

Our registered services include community participation, life skills development, supported independent living, transport assistance,



personal activities and group-based activities.
About the Candidate
At least a Diploma of Community Services or another closely related qualification
A minimum of three years of relevant work experience in community services, disability services or a similar setting
Demonstrated experience assessing participants' needs and coordinating individual support programs
Strong verbal communication and interpersonal skills
Strong written communication skills, including the ability to prepare professional case notes, progress reports and incident records
Ability to communicate respectfully with participants from different cultural and social backgrounds
Current NDIS Worker Screening Check
Current Working with Children Check
Current and unrestricted driver's licence with no suspension or disqualification
Reliable, registered and insured motor vehicle
Satisfactory National Police Certificate with no disqualifying criminal history relevant to the inherent requirements of the position
Ability to provide professional employment references
Availability to work across a flexible roster, including evenings, weekends and public holidays
Ability to work independently and as part of a multidisciplinary team
Work Environment
Permanent full-time employment
Annual salary between $80,000 and $90,000 plus superannuation
Applicable weekend, public holiday, shift, overtime and other penalty rates and allowances in accordance with the Social, Community, Home Care and Disability Services Industry Award ****
