Where you'll be working
Join Western Sydney Local Health District's Chronic and Complex (CAC) Allied Health Service, a dedicated multidisciplinary team committed to helping older people maintain independence, mobility and wellbeing. Based across Mount Druitt and Blacktown Community Health Centres, you'll deliver person-centred care within community and home-based settings, supporting some of Western Sydney's most diverse communities.

Contribute to improving health outcomes for older adults living with chronic and complex conditions
As a Chronic & Complex Physiotherapist Level 1/2, you will provide evidence-based physiotherapy assessment, treatment and education to older adults with chronic, complex and age-related health conditions. Working autonomously and as part of a multidisciplinary team, you will support clients to restore function, improve mobility and achieve their individual goals.
Deliver comprehensive physiotherapy assessments and interventions in clients' homes and via telehealth
Develop, implement and review goal-focused care plans that promote independence and physical wellbeing
Assess falls risk, functional capacity and other factors impacting secure community living
Collaborate with multidisciplinary teams, carers and external service providers to coordinate holistic care
Participate in quality improvement activities, professional development and evidence-based practice initiatives
Requirements
Qualification: Tertiary qualifications in Physiotherapy
Registration: Current general Physiotherapy registration under AHPRA
Selection Criteria
Demonstrated experience and skills in Physiotherapy assessment,
planning and evaluating interventions for clients with chronic and complex health conditions.
Demonstrated understanding of the health and associated care needs of persons with complex, chronic and/or aged conditions residing in the community.
Demonstrated ability in the use of relevant evidence based, best practice guidelines to guide decision making for best client outcomes.
High-level written and oral communication skills, computer competence and ability to use electronic medical records, word processing and databases.
Ability to work independently and as part of multidisciplinary team.
Current Drivers Licence valid within Australia.
