Lead a new model of integrated mental health care.
Communify is seeking an experienced and passionateMental Health Care Partners Team Leaderto lead an exciting new Brisbane North PHN initiative that strengthens collaboration between primary care and specialist mental health services.
This is a unique opportunity to lead a multidisciplinary team delivering recovery-oriented mental health support while helping shape the implementation of an innovative model of care that places people, recovery and collaboration at its centre.
Based at theRedcliffe Medicare Mental Health Centre, you'll combine operational leadership with a reduced clinical caseload, supporting your team to deliver high-quality, person-centred care while building strong partnerships with General Practices and community stakeholders.

Our Medicare Mental Health Centres provide free, multidisciplinary support for adults experiencing mental health concerns through assessment, brief intervention, peer support, care coordination and recovery-focused services.
About the Role
Reporting to theClinical Manager, you will provide operational leadership,



supervision and practice support while maintaining a reduced participant caseload.
You will:
Lead, mentor and support a multidisciplinary mental health team.
Provide clinical supervision, coaching and reflective practice.
Oversee day-to-day service delivery and workforce planning.
Deliver recovery-oriented assessments, care coordination and shared care planning.
Build collaborative partnerships with General Practices and key stakeholders.
Monitor service quality, performance and contractual KPIs.
Drive continuous quality improvement and contribute to program evaluation.
Foster a positive, trauma-informed and recovery-oriented team culture.
About You
You are an experienced mental health professional who enjoys supporting others to thrive while maintaining your own clinical practice.
You will bring:
Qualifications in Social Work, Psychology, Occupational Therapy, Mental Health Nursing, Behavioural Science, Counselling, Community Services or a related discipline.
Demonstrated leadership or supervisory experience.
Experience providing recovery-oriented mental health support for people experiencing complex needs.




Strong skills in biopsychosocial assessment, care planning and care coordination.
Experience working within multidisciplinary teams.
Excellent communication, relationship-building and stakeholder engagement skills.
A commitment to trauma-informed, strengths-based and person-centred practice.
