Assessment and Community Services Manager - Allied Health

Optimum Health Solutions is expanding beyond our clinic walls. We're establishing a dedicated community services arm to reach clients across NSW not currently serviced by our clinic network, and to deliver outreach allied health services to communities across Australia. We need the right Occupational Therapist to lead it.
As Assessment and Community Services Manager, you'll be both practitioner and program architect. You'll carry a caseload, develop the service delivery model, build referral pathways, and coordinate outreach service delivery, all while working directly with our Managing Director to shape the program's direction. Over time, you'll build and lead a team around you.
This role is for an Occupational Therapist with at least five years of post-graduate experience, a solid NDIS background, and the kind of clinical confidence that holds up in complex, unfamiliar environments. You're comfortable working autonomously. You don't need a blueprint. You're motivated by impact over routine.
What you'll be doing
Establishing Optimum's community services arm from the ground up
Conducting community-based assessments across areas of NSW not currently covered by our clinic network
Planning and coordinating outreach services to communities across Australia
Building relationships with referrers, support coordinators, and community partners
Recruiting and supervising practitioners as the program grows
What you'll bring
Tertiary qualification in Occupational Therapy
Current AHPRA registration
5+ years post-graduate experience including community-based delivery
Demonstrated NDIS experience across diverse funding plans
Current driver's licence and willingness to travel for outreach work
