You will join a multidisciplinary team delivering high-level care coordination to HIP clients in the community and outpatient settings.
You will undertake home visits, psychosocial assessments, and help clients access Centrelink, NDIS or VCAT.
Lead case conferences, supervise staff, and contribute to QA and research activities.
