About the Role Join the CO-ACT team and support older people transitioning into community or residential aged care supports.
You will deliver person centred care, build strong team links and maintain safe, high quality care under RN guidance.
What you'll do: Support RN to deliver safe patient care Assess, monitor and report patient status Provide care in line with plans and standards Maintain accurate clinical records Build strong team and partner relationships Communicate with patients, families and services Contribute to smooth care transitions About You You're a caring and capable Enrolled Nurse ready to support older people and deliver safe, person centred care within a collaborative team.
