ABOUT THE ROLE In this dynamic, community-based role, you'll lead the clinical coordination and management of services for a portfolio of Home Care clients.
Working closely with clients, families, and internal teams, you'll ensure care delivery aligns with Support at Home principles, focusing on independence, reablement, and tailored clinical support.
This role involves regular travel across the Central Coast, with visits scheduled based on client needs.
You will: Conduct clinical assessments, onboarding, and ongoing reviews for Home Care clients Developing, reviewing and monitoring personalised care plans and quarterly budgets Provide clinical oversight and respond proactively to changing care needs Deliver hands-on care where required, including wound care, medication management, PAS assessments and health monitoring Support clients with more complex or higher-level clinical needs in collaboration with the broader team Identify risks and implement timely clinical interventions and referrals Mentor and support Home Care Workers, promoting safe,
