Frontline Support Services is seeking a dedicated and experienced Case Manager to join our team in Cairns, QLD. This is a subcontracting role (ABN required), offering an opportunity to make a meaningful impact in the lives of children, young people, and families within our community.
As a Case Manager, you will play a key role in coordinating and delivering essential support services to vulnerable individuals and families while working collaboratively within a supportive and professional team environment.
Key Responsibilities
Conduct comprehensive assessments to identify client needs and develop tailored support plans
Coordinate a range of support services, including referrals, advocacy, practical assistance, and stakeholder engagement
Advocate on behalf of clients to ensure appropriate supports and resources are accessed
Work collaboratively with internal teams and external stakeholders to deliver holistic, client-focused care
Maintain accurate and detailed case notes and documentation in line with organisational policies and compliance requirements
Participate in regular supervision, team meetings, and ongoing professional development
Participate in an afterhours on-call roster shared within the case management team
About You
A relevant tertiary qualification in Social Work, Psychology, Human Services, or a related field (or currently enrolled)




Minimum 2 years' experience in a Case Management or similar role within the community services sector
Strong interpersonal and communication skills with the ability to build rapport and professional relationships
Demonstrated ability to manage complex situations and apply critical thinking and problem-solving skills
Knowledge of child protection legislation, reporting obligations, and trauma-informed practice
Ability to work collaboratively within a multidisciplinary workplace
Excellent organisational and time management skills with the ability to manage a varied caseload
Current driver's licence, valid Blue Card, Yellow Card/NDIS Worker Screening, and ABN
