Clinical Nurse Pathways Consultant (Community Aged Care) - Hybrid
This role would be ideal for someone who has experience and likes developing clinical programs and educating staff - we are looking for that unique person that can do both!

If you have experience with the development of nursing programs, best practice guidelines, supporting and mentoring clinical best practice against clinical pathways - this role would be ideal for you!

Overview
Head Office based in North Brisbane
Hybrid options once embedded into the role - option to WFH and in the office
Full time role – Monday-Friday – versatile with hours to suit your needs
This role doesn’t have any direct reports so no management duties involved
No-on call duties
Reporting and working closely alongside the Quality Manager (who is fantastic and very down to earth!)
Aged Care Community experience is ESSENTIAL (residential aged care experience won’t be considered I'm afraid)
$115k + super (slightly negotiable for the right person and experience)




The previous incumbent was in the role for over 5 years and has recently relocated.

Brief Overview of Duties
Providing coaching and mentoring to individuals and groups of staff
Act as a mentor and give direction to the Community RN's and Clinical Coordinators across the business
Data analysis, creating and updating policies, procedures and processes
Develop clinical pathways with explicit information for staff to access
Ability to identify and manage risks

Requirements
AHPRA Registered Nurse required
Experience in Community Aged Care / Home Care in SAH - ESSENTIAL
Solid clinical judgement required
Must have experience with developing policies, procedures and clinical programs - ESSENTIAL
Experience being able to coach and mentor staff
Rapid-paced role so you will need the ability to multi tasks
You must be IT Savvy to support developing and maintaining the Electronic Client Health Records and web-based applications.
