We are recruiting a Registered Nurse who will work in a small, collaborative clinical team to deliver high-quality clinical care and care coordination across our community programs.
This role will be based in the Illawarra region, with occasional travel to the Shoalhaven region required.
Payrate: $*****/hour + NFP Salary Packaging + KM reimbursement Hours per week: Full time, 38 hours/ week.
We are open to a part-time role for the right candidate.

implement and review car plans and Advanced Health care plans in collaboration with clients and their families/ carers.Support direct care workers through education and guidance to maintain best practice clinical standardsCoordinate services across health providers and community stakeholders to meet participants changing needs.
Essential What we're looking for Registered Nurse with current AHPRA registrationDemonstrated experience working with complex clients in aged care, disability, or community settings Demonstrated clinical leadership and mentoring capabilityCurrent NSW Driver's Licence Current First Aid Certificate Desirable (highly regarded) Advanced clinical assessment and case management experienceKnowledge of Home Care Packages (HCP) and Support at Home programExperience in palliative care, dementia, or end‐of‐life careUnderstanding of Aged Care Quality Standards and clinical governance Why CatholicCare Wollongong?
