Role Overview
As the Clinical Care Coordinator, you'llcoordinate services for clients on Trilogy Care'sRestorative CareandEnd-of-Life pathways.
Working alongside Care Clinicians, you'll support care plan implementation, liaise with clients and providers, and ensure a seamless, person-centred experience, allowing clinicians to focus on delivering clinical care.
What You'll Do
Coordinate client admissions, onboarding, reviews, appointments and transitions across the Restorative Care and End-of-Life pathways.
Maintain regular communicationwith clients, families, representatives and service providers to ensure a seamless, person-centred experience.
Coordinate restorative programs, allied health and community provider referrals, equipment, support services, discharge planning and bereavement follow-up where required.
Coordinate provider services, monitor service commencement, review timeframes and pathway budgets, support My Aged Care processes, assist with funding requests and escape delays.
Working under the direction of the Care Clinician,



complete operational follow-up actions, coordinate outstanding referrals, monitor review dates, escalating client deterioration and support implementation of care plans.
Maintain accurate documentation, case notes, CRM records, clinical outcome data and pathway KPIs.
Comply with workplace health and safetyrequirements and contribute to a safe, supportive and psychologically healthy workplace.
What You'll Bring
Case management experience in aged care or NDIS.
Experience within Support at Home or Home Care programs.
Demonstrated ability to effectively work within a team with positive client outcomes.
Compliance with Trilogy Care pre-employment and ongoing compliance requirements.
Current National Police Check (and international check where applicable).
Evidence of Australian work rights.
