About the Role
We'relooking for an experienced and compassionate Care Partner to join us, working across home and community settings to support our growth in the Southeast region.
This is a dynamic, field-based role—not a desk job.
You'll be out in the community, supporting clients in their homes and local settings, delivering high-quality, person-centred coordination and care.
Your portfolio will focus on clients funded under Supports at Home (Aged Care) ensuring support is personalised, respectful and aligned with funding requirements.
What You'll Be Doing
As a Care Partner,you'llplay a key role in:
Conducting holistic assessments covering physical, psychosocial,culturaland environmental needs
Developing,implementingand reviewingindividualisedcare plans built around each client's goals, wellness, independence,safetyand quality of life
Ensuring reassessments happen at regular intervals and key transition points — hospital discharge, change in condition or change in housing
Being the primary point of contact for clients,familiesand carers and advocating for their rights,dignityand preferences
Overseeing services delivered by third-party providers for clients funded under Supports at Home
Reporting accurately and on time to My Aged Care and other relevant agencies in line with program standards
Growing your client portfolio by sharing the Leora story with clients,representativesand industry partners
About You
To succeed in this role,you'llbring:
A qualification in allied health, aged care,disabilityor a related community field
Community engagement experience,
including networking and stakeholder engagement
Solid stakeholder management and relationship-building skills
Excellent verbal, written and presentation communication
Strongorganisationand decision-making skills, especially in quick-paced environments
A valid Australian DriverLicence(community travel is essential)
A genuine commitment to improving the lives of people living with disability and thoserequiringaged care
You'llalso need a valid NDIS Worker Screening Check (or willingness to obtain one).
