Key Responsibilities
Build genuine, trusting relationships with clients, families, and representatives, becoming their go-to support
Conduct regular care reviews (in person or via phone)



to ensure services continue to meet client needs and expectations
Collaborate with caregivers, families, and health professionals to ensure care plans are personalised and effective
Maintain strong community connections by engaging with local services, health providers, and referral networks
About you
A tertiary qualification in a health-related field (e.g. social work, allied health) with at least 1 year of experience working with seniors and Home Care Packages/Support at Home Packages.
Excellent organisational skills with the ability to juggle priorities and adapt to changing needs
You thrive in a team environment and are willing to offer assistance when needed.
Strong communication skills, both written and verbal, with proficiency in Microsoft Office
A valid Australian Driver's Licence and the flexibility to travel when required
