The
Support Coordinator
will play a vital role in our organization, ensuring seamless coordination of support services for our clients and assisting them in achieving their goals and aspirations. The Support Coordinator will promote a learning focused, valuing and cooperative organization that promotes the development and wellbeing of staff and volunteers.
This role will be responsible for engaging with appropriate service providers and allied health professionals, and monitoring progress toward the achievement of their plan goals. The Support Coordinator is to assist customers to identify community or mainstream supports that can also assist them to achieve their set plan goals. Join TLC as a Full-time employee. . Your duties will include:
Someone who actively seeks to understand the people we support and engage with community to create meaningful relationships;




Someone who follows best practice within the organization ensuring that we maintain integrity around legislation and NDIS guideline;
Someone who has experience in coordinating and assisting in the management of a caseload of complex and potentially at-risk clients.
Someone who can develop and support programs that promote skill development and encourage contribution in leisure, sport, recreational activities in the community. Promote and support clients to access connections and networking opportunities outside of the service system, aligned with individual goal;
Someone who can assist customers to build their capacity around the understanding of their plan and implementation;
Someone who can establish and maintain effective links with the NDIS, TLC programs and other provider services.
