About the Organisation
Based in Melbourne's inner north-west, this provider delivers comprehensive in-home assessments and reviews for older Australians, working collaboratively with clients, families, carers, and service providers to connect people with the right supports at the right time.

The Role
You'll conduct autonomous, person-centred assessments to determine the care and support older Australians need to live safely and independently at home.
Key Responsibilities:
Conduct timely, holistic assessments that support independence and quality of life

Liaise with families, carers, and service providers as key stakeholders in the assessment process
Manage your own assessment schedule in line with Department of Health timelines and quality standards

Full-time or part-time. Your schedule, your choice.
