Mental Health Occupational Therapist (Hybrid Community & Clinic Role)

Mind Plasticity is a leading private mental health practice providing evidence-based services across Sydney and the NSW Central Coast. Working alongside our NDIS-registered sister organisation, Apt Psychosocial Services, we support people experiencing mental health challenges, psychosocial disability and complex functional needs through both mainstream and NDIS-funded pathways.
Our multidisciplinary teams include Occupational Therapists, Clinical Psychologists, Neuropsychologists, Psychiatrists, Mental Health Nurses, Behaviour Support Practitioners, Education Liaison Officers, Psychosocial Recovery Coaches, Allied Health Assistants and Support Workers. This integrated service model creates unique opportunities for clinicians to work across mental health and NDIS settings while receiving strong multidisciplinary support and supervision.
We support adolescents, adults and older adults experiencing a range of mental health, psychosocial and functional challenges, including anxiety disorders, mood disorders, trauma, psychosis, neurodevelopmental conditions, school refusal, social isolation and complex psychosocial disability.
Due to continued growth across both services, we are seeking an experienced Occupational Therapist with a passion for mental health to join our team in a versatile hybrid role.
About the Role
This position offers the opportunity to work across both mainstream mental health and NDIS-funded services, supporting clients in clinic, community and telehealth settings.

The caseload is geographically clustered across Sydney and the NSW Central Coast to minimise travel and maximise clinical efficiency.




Key Responsibilities
Conduct comprehensive functional, psychosocial and environmental assessments
Complete high-quality clinical and NDIS reports, recommendations and documentation
Develop and implement evidence-based occupational therapy interventions
Deliver services in clinic, community, home and telehealth settings
Support clients experiencing mental health conditions and psychosocial disability
Facilitate recovery-oriented, strengths-based goal planning
Collaborate with families, carers, support coordinators, healthcare providers, DCJ, schools and other stakeholders
Contribute to service development, quality improvement and group program initiatives where appropriate
Participate in intake, care coordination and multidisciplinary case discussions

About You
You are an Occupational Therapist with a minimum of two years' clinical experience and a genuine passion for mental health practice.
Bachelor or Master's Degree in Occupational Therapy
Current unrestricted AHPRA registration as an Occupational Therapist




Minimum two years' post-graduate occupational therapy experience
Demonstrated experience working with people experiencing mental health conditions and/or psychosocial disability
Experience delivering services within community-based, clinic and telehealth settings
Strong clinical reasoning, assessment, report writing and communication skills
Demonstrated ability to work independently and effectively within a hybrid work-from-home, community and clinic model
Ability to manage a geographically clustered caseload across Sydney and the NSW Central Coast
Current Australian Driver Licence and access to a reliable vehicle
Current Working with Children Check
Current NDIS Worker Screening Check
Current National Criminal History Check
Professional Indemnity Insurance (for contractors) or ability to obtain
Full Australian working rights
Highly Desirable
Experience working with NDIS participants, particularly those with psychosocial disability
Mental Health Occupational Therapist endorsement (or working towards endorsement)
Occupational Therapy Australia membership
Experience facilitating groups and recovery-oriented programs
Knowledge of contemporary NDIS, psychosocial recovery and mental health frameworks
