Personal Support Solutions - Batemans Bay NSW
As a Support Coordinator at Personal Support Solutions, you will play a crucial role in empowering individuals with disabilities to achieve their goals and live more independent lives. This is a full-time position and involves working closely with participants to coordinate and facilitate the delivery of their NDIS supports.
What you'll be doing
Developing personalised support plans that align with participants' NDIS goals and budgets
Coordinating and monitoring the delivery of supports, ensuring they are meeting participants' needs
Assisting participants to access the services and equipment they require
Building and maintaining strong relationships with service providers, Allied Health professionals and the NDIA
Providing guidance and assistance to participants and their families to help them navigate the NDIS system
Regularly reviewing and updating support plans to adapt to participants' changing needs
Documenting all interactions and maintaining accurate records in line with NDIS requirements
What we're looking for
Relevant tertiary qualifications in disability, community services or allied health field.
Experience in Support Coordination role or similar role, with a strong understanding of the NDIS and its processes.
Excellent communication and interpersonal skills,



with the ability to empathies and build trust with diverse clients.
Strong organisational and time management skills, with the ability to prioritise and multitask
A genuine passion for supporting people with disabilities to achieve their goals and live more independent lives.
A collaborative mindset and the ability to work effectively as part of a team.
A current driver's licence and access to a reliable vehicle.
