About the Role
Join our Integrated Care (IC) Geriatrics team as an Occupational Therapist and help older people maintain independence, build resilience and achieve positive health outcomes within their community. You will deliver comprehensive assessments, coordinate care and support hospital avoidance through restorative, person centred interventions. The GITH service provides care to older adults with complex medical, functional, and/or psychosocial needs within their homes in lieu of an extended or avoidable hospital admission. Occasional weekend work is required. Your scope will include:
Conducting functional and cognitive assessment for older people with complex health needs in the community.
Recommending and arranging patient equipment/ aids, providing patient education, and referring to ongoing support services.
Providing clinical supervision and mentorship to Allied Health Assistants and Occupational Therapy students on placement.
Collaborating within a diverse multi-disciplinary team and liaising with external care teams to support safe and timely discharge, to reduce avoidable hospital re-presentation.
Contributing to service development, quality improvement, and evidence-based practice initiatives within IC and CALHN.
You will be joining a friendly multi-disciplinary team and have an opportunity to learn from other disciplines. Regular structured discipline-specific supervision will be provided, and there will be an opportunity to participate in regular in-services, including discipline-specific professional development.
About You
You are a compassionate and capable Occupational Therapist who delivers person centred care and works confidently within complex community and rehabilitation settings.
AHPRA registration as an Occupational Therapist with no conditions or notations.
Able to deliver evidence-based care for complex client needs
Strong communication and interpersonal skills
Able to manage priorities and work with professional autonomy within a multi-discipinary team
Support quality improvement and service development initiatives
Current SA driver's licence as home-visiting is integral to this role.
