As a Community Care Worker, you'll provide person-centred care and support to aged care clientele living in their own homes within the community.
No two days are the same.
You'll build meaningful relationships while supporting customers with daily living activities that promote choice, dignity and independence.
Your responsibilities will include:
Personal care including showering, dressing and grooming
Medication assistance
Meal preparation and assistance with eating
Domestic assistance including cleaning, laundry and light household tasks
Shopping and transport to appointments or community activities
Social support and companionship
In-home, centre-based and overnight respite care
Monitoring customer wellbeing and reporting changes to the Coordinator or Case Manager
Completing electronic care notes and timesheets in real time
Working safely and collaboratively as part of a supportive team

Experience providing personal care and assistance with activities of daily living




A genuine passion for supporting older people and people living with disability
Excellent communication and interpersonal skills
The ability to work independently while remaining connected to a supportive team
Flexibility to work across varying shifts and customer needs
A Valuable Addition, Yet Not a Must-Have!
Previous experience in Community Care, Aged Care, Disability Support or Home Care
Experience supporting customers living with dementia or complex care needs
Certificate iii Individual Support or equivalent
Australian Drivers Licence– Minimum of a Green P Plate
Proof of COVID-19 vaccination status (or as required by legislation and WMQ policy)
Acomprehensively insured, registered andreliable motor vehicle, with willingness to use your own vehicle for work
Proof of Australian work rights
