From 1 January ****, Sunbury and Cobaw Community Health and Nexus Primary Health have merged to form a new, unified not-for-profit community health organisation, Omnia Community Health. This decision followed an extensive exploration process that confirmed the merger will improve access to safe, high-quality, and sustainable health and community services across the Hume, Macedon Ranges, Mitchell, Murrindindi and Strathbogie local government areas. The new organisation will bring together the deep community connections, shared values and specialist expertise of both entities. Services will span general practice, allied health, disability support, mental health, early childhood, family services, aged care and community wellbeing, delivered from sites in Broadford, Kinglake, Kyneton, Romsey, Seymour, Sunbury and Wallan, with additional outreach into surrounding.
About the Role
District Nurses are responsible for the provision of quality nursing care to clients in their home or clinic-based setting to improve health and well-being and to assist with meeting the health needs of the community.
District Nurses provide services to the community funded through various programs such as Home and Community Care Program for Younger People (HACCPYP), Commonwealth Home Support Program (CHSP), Transport Accident Commission (TAC) etc. Nurses may also be involved in group programs and nurse-led specialty clinics within the community.
To be successful in this role, you will have met the following selection criteria:
Current AHPRA registration as Registered Nurse (Division 1) or Enrolled Nurse
Current clinical experience in community nursing or primary care nursing
Demonstrated knowledge of the Nursing and Midwifery board of Australia's codes and guidelines
Demonstrated knowledge of the Nursing and Midwifery Council's decision-making framework

The District Nurse position is responsible for the provision of quality nursing care to clients in their home or clinic-based setting to improve or maintain their health and well-being and to assist in meeting the health needs of the community. Services are provided to clients funded through the HACC PYP, CHSP, TAC, Post-Acute Care Program (PACP), Hospital in the Home (HITH), Lower Hume Palliative Care (LHPC), Department of Veteran Affairs (DVA), WorkSafe, Commonwealth and State Government funding and other programs as appropriated. Nurses may also be involved in group programs and nurse-led specialty clinics within the community. The employee maybe required to work across multiple Omnia sites in roles within their scope of practice.
