Silverchain is recruiting Registered Nurses to support a chronic disease service across Blacktown and Cumberland LGAs in NSW. The role focuses on assessment, care planning and coordination for people with complex chronic conditions, working closely with GPs, practice teams and specialist services.
The successful candidate will bring nursing qualifications and experience in chronic disease management, health navigation and health literacy, with solid communication and organisational skills.
