We are currently looking for experienced Area Coordinators to join our team
full-time
(Monday to Friday). In this role, you will participate in and oversee the delivery of home and community disability supports for our participants and supervise a team of Support Workers across
both the South and Northside of Brisbane (i.e. Strathpine, Fortitude Valley, Bayside, Acacia Ridge and surrounding suburbs).
Key responsibilities include:
Work directly with participants on the South and Northside of Brisbane to provide community and home care assistance in accordance with individual care plans and NDIS Goals, tasks will include assistance with personal cares, capacity building activities, monitoring and administering medications, organizing and attending medical appointments, promoting social and community inclusion and compliance reporting.
Leadership & Supervision of a team of disability support workers, including training and ongoing mentoring.
Rostering - coordinate the fortnightly allocation of support workers to participants based on participant care needs/hours and staff availability.
Monitor the delivery of support services delivered to participants through review of timesheet submissions, shifts notes, behaviour and incident reports and any other relevant documentation.
Respond promptly to participant feedback, concerns, or complaints, and take appropriate action to address issues and improve service quality.
Ensure care plans and compulsory reports are maintained for each Participants and kept-up-to-date.
Collaborate with external stakeholders including, support coordinators, health professionals, and other relevant parties.
Communicate effectively with participants, their families, and relevant stakeholders regarding care updates, changes, or other relevant information.
Our ideal candidate has:
A Diploma or Degree in Community Services (Disability) or a Bachelor of Social Work or equivalent
A minimum of three years' experience in the Disability sector - working directly with participants with mental health and complex behaviours
Previous experience in the NDIS industry
Previous experience as a Team Leader/Area Coordinator managing large and diverse teams
Previous experience in rostering and stakeholder engagement
High-level of interpersonal and communication skills (written and spoken), with the ability to communicate effectively with staff at all levels of the organisation
Full working rights in Australia -
please note this role is not suitable for student visa holders.
Valid driver's license with own vehicle with comprehensive car insurance (
frequent driving is required)
