We are seeking an experienced
Complex Care Team Leader
to join our growing team across
Colac and Geelong
. This leadership role is ideal for someone who is passionate about supporting staff, driving quality outcomes, and delivering exceptional care to participants with complex support needs.
About the Role
As the
Complex Care Team Leader
, you will work within the SIL houses to provide day-to-day operational leadership to a team of Disability Support Workers delivering supports to participants with complex physical, psychosocial, behavioural and mental health needs across community.
Working under the guidance of the
Program Coordinator
and
Program Manager
, you will mentor staff, monitor service quality, support participant outcomes, and ensure services are delivered in accordance with NDIS Practice Standards, organisational policies and best-practice approaches.
This is a hands-on leadership role that combines staff support, participant oversight, quality assurance and operational coordination.
Key Responsibilities
Lead, mentor and support Complex Care Disability Support Workers.
Provide day-to-day operational support to staff across complex participant supports.
Promote a positive, professional and person-centred team culture.
Ensure participant supports are delivered safely, consistently and in line with individual support plans.
Support the implementation of Positive Behaviour Support Plans, risk management strategies and restrictive practice requirements.
Conduct regular staff check-ins,



coaching and performance conversations.
Assist with onboarding, orientation and ongoing staff development.
Review participant documentation, progress notes and incident reports for quality and compliance.
Complete participant reviews, quality audits and service monitoring activities.
Conduct medication audits and compliance checks where required.
Work collaboratively with the Program Coordinator and Program Manager to ensure effective service delivery.
Liaise with Behaviour Support Practitioners, allied health professionals, families and external stakeholders.
Assist with workforce planning, shift coverage and operational coordination.
Identify opportunities for service improvement and contribute to continuous quality improvement initiatives.
Escalate participant, staffing, clinical or operational concerns appropriately.
Current Service Needs
We are seeking a Complex Care Team Leader to support our expanding complex care services across
Colac and Geelong
.
This role will:
Provide leadership and mentoring to Complex Care Support Workers.
Support participants with high and complex support needs, including psychosocial disability, mental health, Behaviours of Concern (BOC), forensic presentations and complex physical care where required.
Work closely with the Program Coordinator to ensure participant goals and support outcomes are achieved.
Monitor service quality, compliance and staff performance across allocated participants.
Participate in an on-call leadership roster where required.
What You'll Bring




Previous leadership or senior experience within the disability, community services or healthcare sector.
Demonstrated experience supporting participants with complex care needs.
Strong understanding of Positive Behaviour Support, trauma-informed practice and recovery-oriented approaches.
Experience supporting participants with Behaviours of Concern, psychosocial disability, mental health and/or complex clinical needs.
Excellent leadership, coaching and communication skills.
Strong organisational and time management abilities.
Ability to confidently manage competing priorities and make sound decisions.
High level of professionalism, integrity and accountability.
A passion for developing staff and delivering exceptional participant outcomes.
Requirements
Current First Aid & CPR Certificate.
NDIS Worker Screening Check.
Valid Australian Driver's Licence.
Reliable and insured vehicle.
Medication Administration competency.
Certificate IV or higher in Disability, Individual Support, Community Services, Mental Health or a related field (preferred).
Australian working rights.
100 points of identification.

Employment is governed by the
Social, Community, Home Care and Disability Services Industry Award **** (SCHADS Award ****)
and Infinity Support Services policies.
Salary will be negotiated based on experience and qualifications.
