Upward Life Family Practice is seeking an experienced Chronic Disease Management Nurse or Nurse Practitioner to join our growing team on either a full-time or part-time basis. School hours will be considered for the right applicant.
You will lead chronic disease management services, coordinate care with GPs and a multidisciplinary team, and deliver holistic, patient-centred care in a supportive workplace. Immunisation certification and Best Practice software experience are desirable.
