Your caseload will be arranged to be geographically close to your home.
About the Role
This is a full time permanent position, with flexibility for part-time applicants.
As a Community Occupational Therapist, you will deliver high-quality, person-centred care through home visits across the Central Coast. We offer a highly flexible caseload that can be tailored to your clinical interests, experience, and preferred style of practice.
Whether your passion lies in NDIS/disability care or aged care, we will structure your caseload to suit you:
NDIS / Disability Focus: Deliver Functional Capacity Assessments (FCAs), SIL and SDA assessments, assistive technology prescription, home modifications, and therapy aimed at building independence and capacity.
Aged Care Focus: Support older adults under the Support at Home program to remain living independently, through assessment, intervention, and preventative care.
We aim to provide a diverse but clinically meaningful caseload, ensuring you are both challenged and supported in your role.
Key Responsibilities
Manage a community-based caseload with structured triage to ensure it aligns with your scope of practice
Conduct comprehensive assessments in clients' homes and community settings
Prescribe assistive technology and complete home modification recommendations
Deliver therapy and capacity-building interventions in the home or clinic environment
Complete high-quality clinical documentation, reports, and stakeholder communication
Liaise with families,



support coordinators, and other health professionals to achieve optimal outcomes

About the Practice:
Your Physio Advantage is a growing allied health team of 32(8 OT's). With the systems and opportunities of a large provider but the team work and support of a small practice. Our outstanding growth has been too due to our exceptional team, with validated feedback showing we are in the top 10% of workplaces to work for! We operate across the Hunter and the Central Coast region.
