This is Fixed Term Full Time appointment commencing November **** until 28/06/2027 at 76 hours per fortnight, with the possibility of extension and/or permanency.

TheOlder Adult Health Hubsare a new model of care, providing safe and quality care for older adults through integrated, community-based intervention.
This specialist-led older adult hub delivers and integrates with services that focus on proactive and responsive care, risk prediction and early intervention.
TheSMHS Older Adult Health Hub:
Optimises older adult independence through maintaining and improving function, growing health literacy, supporting choice autonomy and scaffolding and supporting carers
Provides coordinated, comprehensive multidisciplinary care
Supports liaison between primary care and SMHS services
SMHS delivers quality, safe and effective hospital and health services within a catchment area stretching more than **** square kilometres across the southern half of Perth.

We are seeking aSenior Pharmacistto join our team.
Position Description:
Currently we are seeking aSenior Pharmacistto join our team.
In this role you will work as part of the multidisciplinary team to provide specialised Geriatric Medicine clinical pharmacy services, which includes monitoring of the patient's total medication profile, providing drug information and undertaking teaching and research.
The positions manages an advanced clinical caseload,



as well as contributes to care coordination and manages effective communication with primary care and referrers.
We are looking for someone who can contribute to the service leadership and support the development of this exciting new Older Adult Health Hub service.
