DPV Health is seeking an experienced Support at Home Care Partner to collaborate with service teams in delivering versatile, personalised and culturally appropriate care plans for participants and carers. You will engage new participants, develop care plans, monitor budgets and connect clients with local services, while supporting families and carers in decision-making.
This role requires experience in community aged care and ACAT/ RAS environments, with a strong focus on ethics and quality
