We are currently seeking a compassionate and reliable part-time Care Partner to support our Aged Care and Department of Veterans' Affairs (DVA) clients in the Shepparton region.
This is a case management-focused role where you will coordinate and monitor care services, ensuring clients receive high-quality, person-centred support tailored to their goals and needs. Beginning with approximately 25 hours per week with opportunity to move full-time within 3 - 6 months, this position is ideal for someone who is passionate about making a positive impact in the community and can communicate effectively with clients, families, and support networks.
Key Responsibilities
Coordinate and monitor care plans for Aged Care and DVA clients in the Shepparton region
Conduct regular client visits and assessments to review care needs, goals, and wellbeing
Liaise with clients, families, healthcare providers, and internal teams to ensure cohesive support
Update and maintain accurate case documentation and records in line with organisational and compliance requirements
Identify and elevate any risks, concerns, or changes in client status to the appropriate team members
Support clients to access and navigate community services and resources




Ensure all case management activities align with relevant policies, DVA guidelines, and aged care standards
Key Requirements
Previous experience in case management, aged care, disability support, ora related community services role
Strong understanding of person-centred care and the ability to tailorsupport to individual client needs
Effective verbal and written communication skills, with the ability to buildtrust and rapport with clients
Demonstrated ability to work independently, prioritise tasks, and manage acaseload
Reliable, professional, and compassionate approach to supporting clients inthe community
Current driver's licence and ability to travel within the Shepparton region
Relevant qualifications (e.g., in health, community services, or social work) are desirable
