All Care Nursing (ACN) is currently partnering with an aged care provider to recruit an experienced Clinical Care Coordinator for their well-established 70-bed home located in the Foster, South Gippsland VIC.

The provider is a community based, not for profit, residential aged care service. Providing ageing in place residential care services as well as short-term respite care for the local community. The 70-bed home is fully accredited to Government standards.

Key responsibilities
Ensuring high-quality professional and clinical care for all residents.
Supporting the Care Manager in oversight of care operations, HR activities, staff supervision and development.
Coordinating and monitoring care processes, assessments, AN-ACC assessments, risk management, and clinical governance
Providing mentoring, coaching, and on-the-floor leadership to nursing and care teams.
Delivering and supporting orientation, staff education, clinical training, and continuous improvement.
Acting as Infection Prevention and Control (IPC) Lead or supporting that function as required.




Liaising with doctors, allied health providers, families, contractors, and external services.
Acting on behalf of the Care Manager in their absence
Current AHPRA registration as Registered Nurse.
2-4 years of relevant work experience for this role
Experience in aged care clinical leadership roles.
Working knowledge of Strengthened Aged Care Quality, legislation, IPC, and quality frameworks.
Clinical assessment skills and understanding of Ageing in Place.
Strong communication, interpersonal and documentation skills.
Demonstrated initiative, autonomy, and ability to work in a team.
Strong organisational, time management and prioritisation skills.

Previous experience as an Infection Prevention & Control Lead
Previous experience with AN-ACC Assessments and funding
