About the role
We are seeking a dedicated and compassionate Disability/Aged care Support Coordinator to join our team at Enable Living Supports in Ballarat, Victoria. This is a full-time position that plays a vital role in coordinating and delivering high-quality support services to our clients. As Disability/aged care Support Coordinator, you will be instrumental in ensuring our clients receive appropriate, timely, and person-centred support that enables them to achieve their goals and maintain their independence. This role is essential to our operations and directly impacts the wellbeing and outcomes of the people we support.
Key responsibilities
Coordinate and deliver disability support services to clients, ensuring all support plans are implemented effectively and consistently
Assess client needs and develop personalised support plans in collaboration with clients, their families, and other relevant stakeholders
Maintain detailed and accurate records of client progress,
support provided, and any incidents or concerns that arise
Monitor and evaluate the effectiveness of support services, making adjustments as needed to ensure optimal client outcomes
Communicate regularly with clients, families, and multidisciplinary teams to ensure coordinated and comprehensive care
Support clients in accessing community resources, services, and opportunities that promote independence and social inclusion
Provide practical assistance with daily living activities, personal care, and household tasks as outlined in individual support plans
Identify and report safeguarding concerns, ensuring compliance with duty of care obligations and reporting procedures
Contribute to team meetings and provide feedback on service delivery and client outcomes
Maintain professional boundaries whilst building trusting and supportive relationships with clients
What we're looking for
Certificate III or IV in Disability Support Services, or equivalent qualifications and demonstrated experience in support work
Proven experience working with people with disabilities in a community or aged care setting
Strong understanding of person-centred care principles and commitment to upholding client dignity and independence
Excellent communication and interpersonal skills, with the ability to build rapport with clients from diverse backgrounds
Ability to work both independently and as part of a collaborative team
Strong organisational and time management skills, with attention to detail in record-keeping and documentation
Empathy, patience, and a genuine passion for supporting people with disabilities
Current Working with Children Check (or willingness to obtain if supporting younger clients)
Valid driver's licence and access to reliable transport
First Aid and CPR certification, or willingness to obtain
Flexibility to work various shifts, including early mornings, evenings, and weekends as required
