Success factors & skills required are:
Providing holistic aged care services through assessment, planning, implementation and evaluation of resident care support and services
Provision of best practice clinical care
Medication management, wound management, pain management, continence management, dementia care and palliative care
Accurate and contemporaneous documentation
Providing mentoring, guidance and support to the team
Ability to work as part of a dedicated multidisciplinary team
Liaising with external support services
Ability to communicate effectively with residents, families and NDC team members
Requirements:
AHPRA Registration
2x COVID 19 Vaccination plus 1x Booster Shot (total of 3)
NDIS Workers Screen Clearance
Individuals in these roles are required to comply with statutory and government requirements, such as public health directions, in place from time to time including requirements with respect to NDIS Worker Screening Checks and influenza and COVID-19 vaccinations.
