Launch Housing is seeking a permanent part time Intensive Case Manager (60.8 hours per fortnight) based at Dandenong. You will work in the Supportive Housing team to help rough sleepers progress toward housing, improve health, and connect to community supports through a multidisciplinary approach.
You will develop flexible engagement strategies, build client plans, and collaborate with partner agencies to deliver high-quality, person-centred services while meeting reporting standards and safety
