About the Role
As a Service Delivery Coordinator, you will oversee the delivery of high-quality, person-centred disability support services, ensuring participants receive supports aligned with their goals and NDIS requirements. You will lead and support staff, manage participant onboarding, service quality, incidents, while building strong relationships with participants, families, and stakeholders. The role focuses on continuous improvement, effective risk management, and maintaining the highest standards of care and documentation.
Key Responsibilities:
Participant Intake & Onboarding
Assist with participant onboarding.
Prepare and maintain Service Agreements, participant profiles, Rosters of Care and onboarding documentation.
Liaise with participants, families, Support Coordinators, guardians and allied health professionals to ensure a smooth transition into services.
Service Delivery & Quality
Oversee day-to-day service delivery, ensuring supports are person-centred and aligned with participant goals.
Monitor service quality, participant outcomes and documentation compliance.
Conduct case reviews, site visits and team meetings.
Supervise Team Leaders and provide coaching to improve service delivery and documentation standards.
Foster a culture of accountability, quality, continuous improvement and person-centred care
Participant Support & Stakeholder Engagement
Build solid relationships with participants and key stakeholders.
Resolve participant concerns, complaints and service issues promptly.
Ensure Support Plans, Behaviour Support Plans and Risk Assessments are implemented effectively.
Incident, Risk & Compliance
Review incidents, oversee follow-up actions and identify trends to drive continuous improvement.
Manage participant risks and escalate significant incidents or safeguarding concerns to the Compliance Manager.




Support implementation of corrective actions and risk mitigation strategies.
What we're looking for
Relevant qualifications in disability support, community services, nursing, social work or a related human services field.
Proven experience in a service delivery, coordination or leadership role within the disability or community services sector.
Sound knowledge of the NDIS Practice Standards, NDIS legislation, relevant frameworks, participant planning processes and quality requirements.
Excellent interpersonal and communication skills, with the ability to build strong relationships with participants, families, staff and stakeholders.
Strong organisational and time management skills, with the ability to manage multiple priorities and competing demands.
Demonstrated commitment to person-centred practice and a strong understanding of the principles of choice, control and dignity for people with disability.
Solid understanding of participant documentation, record-keeping requirements and compliance obligations.
Excellent written and verbal communication skills, with strong attention to detail.
Proficiency in Microsoft Office and experience using client management systems or electronic records.
Current First Aid and CPR certification.
Current driver's licence and access to a reliable, registered and insured vehicle.
Current NDIS Worker Screening Check, Working with Children Check and National Police Clearance (or willingness to obtain one).
Unrestricted right to work in Australia.
