About Reach Social Supports
Reach Social Supports was founded in **** to address a growing absence in the disability sector of high-quality community focused NDIS supports in the Gippsland region. Reach now provides social group support, one-on-one support, support coordination and more to the NDIS communities of Gippsland, and Melbourne. We strongly advocate for the importance of community and social inclusion, and bring this into everything we do. Our services are focused on the client: assisting them to achieve their personal goals and to be a part of an inclusive community of like-minded individuals who are there for each other.
NDIS Support Worker Roles
The Support Facilitator will be responsible for a broad range of tasks including 1-to-1
mentoring
and
support
, working with clients in a
group support
capacity, and even the option for some day trips with clients. The role will report to the team leader and management team.
This role is suited to someone who has a high degree of initiative and the ability to deal with a broad range of tasks. Reach also recognises the tendency for some people to only apply to roles where they meet 100% of the requirements. However,

Pay based on SCHADS award rate + super

Key responsibilities for the Support Facilitator:
Delivering one-to-one services including accompanying clients on errands and appointments
One-to-one support including community access, emotional support and care, assistance with daily living activities
Delivering group supports in a centre, assisting in the delivery of activities and outings
Provide direct support services in accordance with individual care plans
Assist with skill development for independent living, recreation, learning, and community access
Advocate on behalf of the individuals and families to ensure services are relevant, flexible and accessible
Maintaining client records with valuable note taking and document maintenance
Must hold or be willing to obtain:
Valid Driver's Licence with no restrictions
Current First Aid & CPR Certificate (or willing to obtain)
