About Sensible Care
Sensible Care is a values-led provider of Support at Home services — the program that replaced Home Care Packages from 1 November ****. We deliver individualised, holistic, and ethical care to older Australians, and we're growing quickly. We're looking for people who want to do this work properly.
The Clinical Care Partner brings clinical leadership to a caseload of Support at Home participants — people living with chronic conditions, wounds, cognitive change, or approaching the end of life, who want to stay in their own homes for as long as possible.
You'll hold clinical oversight across your caseload: assessing needs, managing risk, and shaping care plans that are clinically sound and genuinely reflect what each person wants their life to look like. You'll work alongside Care Partners, nurses, allied health professionals, general practitioners, specialists, families, and carers — and you'll be the person your colleagues turn to when something clinical needs a second opinion.
It's a role with real autonomy. You'll manage your own diary, make your own clinical judgements, and see the difference your decisions make in people's lives, week after week.
In this role, you will
Complete comprehensive assessments that identify health risks, clinical needs, and the interventions most likely to help.
Work with participants to build and review care plans that reflect their goals, preferences, cultural needs, and clinical requirements.
Provide clinical guidance for participants with complex needs — chronic disease management, wound care, medication support, continence, falls prevention, and palliative care.




Be the clinical point of contact for Care Partners and care workers in your region, offering advice, support, and oversight of practice.
Coordinate with general practitioners, specialists, hospitals, pharmacists, and allied health providers so care stays joined up, particularly through transitions in and out of hospital or respite.
Monitor wellbeing and clinical outcomes over time, picking up changes early and acting on them.
Support participants and their carers to make informed decisions through education, advocacy, and supported decision-making.
Work with participants to plan and manage their individualised budgets, so services match assessed needs and make good use of available funding.
Keep clear, accurate clinical documentation in our care management system.
Identify, report, and isolate clinical incidents, risks, and concerns.
Contribute to quality improvement and reflective practice, and keep service delivery aligned with the Aged Care Act ****, the Strengthened Aged Care Quality Standards, and Support at Home program requirements.
What you'll bring
Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
Experience in aged care, community nursing, or a comparable setting, and confidence assessing complex needs.




Well-developed assessment skills and the ability to problem-solve creatively when care gets complicated.
An understanding of community-based care and consumer-directed care principles.
Familiarity with the Support at Home program, the Quality Standards, and the rights of older people (or the appetite to get across them quickly — we'll support you).
Warmth, strong communication, and cultural competence.
Good organisational skills and the ability to work independently.
An Australian driver's licence.
A National Police Check, or willingness to obtain one, and full work rights in Australia.
Also great, but not essential
Care management or case management experience under Home Care Packages or Support at Home.
Post-graduate study or credentialling in gerontology, wound management, palliative care, dementia care, or chronic disease management.
Experience supporting and supervising care workers in the community.
Experience with a care management system.
Additional languages spoken in the communities we serve.
