Care Partner - Support at Home Program/HCP
Help older Australians get the most from their care
Elka Home Care supports older Australians to live safely, independently and confidently in their own homes.
We are looking for an experienced Care Partner to take ownership of a portfolio of Support at Home clients. You will be the trusted link between each client, their family, our service team and external providers.
This is a client portfolio and case-management position—it does not involve providing hands-on personal care.
About the role
As a Care Partner, you will manage the client relationship from onboarding through to ongoing service delivery, care-plan reviews and changing care needs.
Success in this role means your clients feel known and supported, their services run reliably, their care plans remain current and their available funding is managed responsibly.
Your responsibilities will include:
Managing your own portfolio of Support at Home clients
Completing client onboarding, assessments, service agreements and support plans
Building trusted relationships with clients, families and representatives
Coordinating support workers, allied health professionals and other service providers
Conducting regular reviews and client home visits
Monitoring client budgets, expenditure and service utilisation
Helping clients understand their funding, services and statements
Identifying underutilisation, changing needs and opportunities to improve client outcomes
Responding promptly to service issues, incidents, feedback and complaints
Maintaining accurate case notes, care plans and client records
Working closely with rostering, intake, finance, clinical and service-delivery teams
Supporting client retention and continuity of care
Ensuring services meet Elka's policies, safeguarding requirements and relevant aged care standards
About you
You are organised, responsive and comfortable taking ownership. You can build genuine relationships while also managing budgets, documentation, deadlines and competing priorities.
You will ideally have:
At least 12 months' experience in home care case management, care coordination or a similar client portfolio role
Practical knowledge of Home Care Packages or the Support at Home program
Experience completing assessments, support plans and regular client reviews
Confidence managing client funding, budgets and service utilisation
Solid communication and relationship-management skills
Good judgement and the ability to resolve issues calmly and professionally
Excellent organisation, documentation and time-management skills
Confidence using care-management systems, CRMs and Microsoft Office
A relevant qualification in community services, aged care, nursing, allied health or a related discipline
Ability to work autonomously and as part of a team
Enjoy a fast paced decision making environment
A current National Police Check
A valid Australian driver's licence and reliable vehicle
