HammondCare is seeking a Dementia Support Coach to deliver hands-on, evidence-based support for people living with dementia and those who care for them across community and residential settings.
You will provide tailored education from DREAM modules, through group sessions and individual mentoring, working with staff, leaders and stakeholders to ensure context-specific, person-centred care. This fixed-term role runs until June **** and can be based in Chermside, QLD or Malvern, VIC.
