We're currently seeking an experienced and compassionateSupport at Home Care Partnerto join our dedicated team in Coffs Harbour.
This is an exciting opportunity for someone with experience managing Home Care Packages and the new Support at Home Program, who thrives on building relationships, coordinating quality services, and empowering people to achieve their goals.
As a Support at Home Care Partner, you'll manage a caseload of up to 60 clients across varying package levels, delivering personalised, person-centred care coordination and case management.
Working alongside nurses, support workers, families and community providers, you'll play a vital role in ensuring our clients receive the support they need to remain independent and connected to their community.

About the Role
As a Support at Home Care Partner, you will:
Act as the primary contact and advocate for clients and their families
Develop personalised, goal-focused and consumer-directed care plans in partnership with clients
Coordinate services that align with client goals, preferences and allocated budgets
Monitor and review care plans to ensure quality outcomesWork collaboratively with nurses, support workers and service providers
Liaise with ACAT, healthcare professionals, referral agencies and community organisations
Conduct risk assessments and support client wellbeing and safety
Manage budgets and funding effectively while ensuring compliance with program requirements
What You'll Bring
A minimum Certificate IV in Aged Care, Community Services, Community Care and/or Case Management (Diploma desirable)
At least 2-3 years' experience in care coordination, case management or a similar role
Experience working with Home Care Packages and/or Support at Home programs
Strong understanding of the Aged Care Act, Aged Care Quality Standards and associated regulations
Demonstrated experience managing client budgets and financial information
Proven ability to assess risk and support client safety and wellbeing
Excellent communication, relationship-building and organisational skills
The ability to manage competing priorities while maintaining a high level of client care
A current National Police Check
Current First Aid and CPR certification
