Key Responsibilities
Assist with CEO and team to work on Support at Home application documents, go through accreditation audit process
Contribute to the oversight of clinical governance, risk management, and quality improvement initiatives
Review clinical incidents, trends, audit outcomes, and consumer feedback relating to dementia care
Support development of dementia-friendly and person-centred care models
Participate in committee meetings and strategic governance discussions
Support organisational compliance with Aged Care Quality Standards and clinical governance obligations
Promote continuous improvement and positive consumer outcomes across home care services
What We Are Looking For
To be successful in this role, you will have:
Experience working within or demonstrated understanding of the Strengthened Aged Care Quality Standards framework
Relevant tertiary qualifications in Nursing, Medicine, Allied Health, Psychology, Public Health, Research, or related field
Strong understanding of dementia care frameworks and aged care quality standards
Experience in governance, advisory committees, research, or quality improvement initiatives
Knowledge of contemporary aged care reforms and community care models
Excellent communication, stakeholder engagement, and analytical skillsCommitment to person-centred, evidence-based care practices
Desirable
Tertiary qualifications in Social Work, Aged Care, Disability Services, Community Services Coordination, Nursing or Allied Health discipline with a current AHPRA registration, or equivalent experience
Experience in Support at Home, Home Care Packages, or community aged care
Experience working in case management or care management in either Home Care Packages/Support at Home Program or similar
Clinical background in geriatrics, nursing, psychology, or allied health
