Fixed Term Part Time (7 months, 30 weeks) | 32 Hours per week YP2 - YP9 | $***** to $***** per hour About The Role Grampians Health - (Horsham & Edenhope campuses) is currently seeking applications for the **** Transition to Practice (Graduate) Program.
Key requirement for this position is that you have NOT been employed or involved in a **** graduate program.

Prospect to submit preferences of where you will be allocated prior to the conclusion of their Graduate transition programcomprehensive 4-day orientation program and a further 6 professional development daysSupernumerary time at the commencement of each rotation and regular Graduate debrief sessionsDevelop self-confidence, critical thinking, decision-making, clinical skills, and clinical competence as part of the multifaceted role of the Registered NurseFacilitate practical and theoretical learning opportunities Key Responsibilities Works collaboratively with the Nurse Unit Manager and the team to support and enhance the core business of the designated clinical areaActively participate in the Graduate Nurse Program, including completion of all requirements outlined in Graduate Orientation.Be responsible for planning, implementing, and evaluating evidence-based nursing care in collaboration with the multidisciplinary team to achieve optimal healthcare outcomesPractices independently and interdependently assuming accountability and responsibility for their own actions and the delegation of care to other healthcare workers as requiredProvide high-quality, comprehensive,
and evidence-based patient-centred care whilst acting as a resource and role model to other staff Skills & Experience Establishes a compassionate environment by providing emotional, psychological, and spiritual support to staff, patients/residents their families, and carers.Maintains relationships with the health care team by communicating information, building rapport, and participating in team problem-solving.Participates and engages in the annual performance review and professional development plan in accordance with the Grampians Health Staff Development Program.
