Cre8Away is seeking a Support Coordinator to help NDIS participants achieve goals by connecting them with community and mainstream supports.
You will coordinate plans, negotiate services, and promote participant choice while maintaining accurate records and collaborating with a team. A Certificate IV in Disability or relevant experience is required, and safety checks are essential. Monday–Friday office hours apply.
