You will care for clients across age groups with complex wound care, palliative care, medication administration and chronic disease management.
Your duties include initial assessments, ongoing care coordination, and care planning within a multidisciplinary team to ensure optimal client outcomes.
The role requires immunisation readiness and current AHPRA registration.
