About the role
As part of Public Health and Primary Care Unit, Primary Care teams deliver services to the multi-disciplinary Primary Care program area. The Primary Care Intake Role supports these teams by coordinating the screening and prioritisation of new referrals. Working closely with internal teams and external providers, the role determines service suitability, confirms priority for care and escalates concerns when required to ensure protected, timely and high-quality consumer experiences. Through holistic assessment of referrals and enquiries, the Intake role ensures consumers are connected with the most appropriate services across Barwon Health and receive person-centred, goal-directed care aligned with their individual needs.
Working within their scope of practice, the Intake role identifies additional services that may benefit consumers and provides information regarding available services, fees, eligibility criteria, and expected waiting times.
The Intake role works collaboratively with Primary Care teams, Information & Access services, and site-based clinical and administrative staff to support effective referral management and care coordination. Maintaining a broad understanding of services across Barwon Health, the Intake Clinician develops strong relationships with key stakeholders to facilitate access to appropriate care pathways and ensure consumers receive the right care at the right time.
Key responsibilities
Provide consumer-centred care and referral management by conducting comprehensive screening, identifying client needs, prioritising referrals, and ensuring timely access to appropriate services in line with clinical guidelines and scope of practice.
Deliver high-quality, safe, and effective services by adhering to Barwon Health policies, procedures, clinical standards, health literacy principles, and relevant legislative and professional requirements.
Collaborate with multidisciplinary teams and stakeholders to support coordinated care, effective communication, service accessibility, and positive consumer outcomes.
Contribute to service improvement and career development through participation in quality improvement activities, reflective practice, evidence-based care, and ongoing learning to enhance service delivery.
What we're looking for
Bachelor's, Master's, or Doctoral Degree in Nursing or an Allied Health discipline, with current AHPRA registration (where applicable) and experience consistent with the relevant Award/Agreement classification requirements.
Demonstrated clinical expertise, independent practice capability, and commitment to ongoing professional development, reflective practice, and evidence-based care.
Strong communication, interpersonal, and relationship-building skills, with the ability to collaborate effectively with multidisciplinary teams, resolve conflicts,
influence outcomes, and facilitate open discussions.
Demonstrated proficiency in information technology systems, ability to work independently and collaboratively, and commitment to delivering inclusive, culturally safe care aligned with Barwon Health values.
