About the role
We are seeking an experienced Community Engagement Consultant to deliver high-quality, client-centred intake experience across Aged Care programs and services. In this role, you will act as the first point of contact for clients, families and referrers, providing timely personalised service information while conducting eligibility screening, intake and assessments in line with organisational policies and procedures.
Salary:
$90,****** per annum, based on skills and experience plus 12% superannuation (pro‐rated for part‐time employment)
Key Responsibilities
Provide a prompt response to calls/queries and provide targeted information to prospective clients, carers, advocates and referrers in relation to Brotherhood's Aged Care programs and services.
Update and maintain accurate information/ records in relevant database/systems in a timely manner
Work in a collaborative manner with other team members to achieve optimal and responsive outcomes for clients. This may include initial visits to prospective clients with Case Managers to ensure seamless transition to the program
Foster, advocate and implement a client service focus and practice
Support business development, marketing and relationship building activities

What we are looking for
We are seeking a motivated and experienced community engagement consultant who is passionate about driving continuous improvements and supporting BSL's mission.
Key skills and experience
Demonstrated experience in Home Care Packages/Support at Home is preferred
Relevant tertiary qualifications in Aged Care Case Management, Nursing, Allied Health or Social Work
Demonstrated high degree of initiative and ability to identify and resolve problems and/or make appropriate recommendations
Excellent interpersonal and communication skills with the ability to build effective relationships and liaise across all levels both internally, externally and with people from diverse backgrounds
Knowledge of The Care Manager software
Demonstrated client service skills and commitment to the provision of high-quality client service
