Registered Nurse | Agency Aged Care & Community Care
We are working with a local community care organisation to appoint a dedicated and compassionate Care Partner to support older Australians living at home through Support at Home packages.
This role is ideal for experienced Community Enrolled/Registered Nurses seeking a rewarding transition into case management. You will build robust client relationships, lead new client onboarding, coordinate personalised care plans, and work closely with care teams to ensure high-quality services are delivered.
Key Responsibilities
Develop and maintain trusting relationships with clients and families.
Conduct onboarding, care reviews, and ongoing coordination using a wellness and reablement approach.
Create tailored care plans and service agreements.




Identify opportunities to expand services through client needs and community connections.
Promote services to support growth and enhance outcomes.
Work collaboratively with Caregivers and provide constructive feedback.
About You
Current AHPRA registration
Experience in aged care, case management, or Home Care Packages.
Comprahensive knowledge of Support at Home framework.
Experience in sales, growth, or business development is desirable.
Strong interpersonal and communication skills.
Confidence using Microsoft Office and CRM systems.
Valid Australian Driver's Licence and willingness to travel across Brisbane North & Moreton Bay.
