Support Coordinator - Casual (2-3 Days per Week)

Raya Healthcare is seeking an experienced Support Coordinator to join our team on a casual basis, supporting NDIS participants across Melbourne's northern suburbs to achieve greater independence, connection and choice in their lives.

Position Overview
Working 2 to 3 days per week from our Thomastown hub, the Support Coordinator will manage a caseload of NDIS participants, ensuring their plans are effectively implemented and their goals are actively supported. This role involves regular travel across Melbourne's northern suburbs to meet participants directly, and offers genuine opportunity for growth as our Support Coordination service expands.
Key Responsibilities
Work directly with participants to understand their NDIS plans, goals and support needs
Connect participants with appropriate funded, mainstream and community supports
Build participants' capacity and confidence to exercise choice and control
Monitor plan budgets and support utilisation, identifying funding gaps or risks early
Provide support to participants during points of crisis or service disruption
Liaise with LACs, NDIA planners, allied health professionals and other providers
Maintain accurate and timely case notes and reporting in ShiftCare
Ensure all supports align with the NDIS Code of Conduct and Practice Standards
Selection Criteria
Demonstrated experience in NDIS Support Coordination or a closely related case management or community services role
Certificate IV in Disability, Community Services, Mental Health or related field (Diploma or higher highly regarded)
Sound working knowledge of the NDIS Act, Practice Standards and participant funding categories
Current NDIS Worker Screening Check, or willingness to obtain one
Current Australian driver's licence and access to a reliable, comprehensively insured vehicle
Current First Aid and CPR certificate
Robust communication, negotiation and relationship-building skills
A person-centred approach characterised by empathy, patience and initiative
Experience with ShiftCare or comparable case management software
Experience in Specialist Support Coordination (Level 3) or Psychosocial Recovery Coaching
Working with Children Check
