About the Role
Formed in 1882, Royal Prince Alfred Hospital (RPAH) is a major metropolitan tertiary referral hospital and a principal provider of specialist healthcare. It remains one of the nation's most respected hospitals, with a distinguished history serving the health needs of local, statewide, national and international communities.

The Planned Care service delivers short term (up to 12 weeks), innovative, integrated care that supports people with complex social and medical needs to live well in the community.

The PCBH service targets adults who are at risk of hospitalisation and aims to reduce avoidable hospital presentations and duplication of services.

The Care Coordinators work within a multidisciplinary community-based team that delivers individualised support and coordination of a person's health and social support needs.
